Do not include passwords, Social Security numbers, banking information, claims files, or confidential insurance documents.

What happens next

  1. We acknowledge and classify the request.
  2. We verify identity and, for an agent, authority to act.
  3. We search relevant systems and apply lawful exceptions.
  4. We complete the action or explain the result and appeal option.

California requests are generally answered within 45 calendar days, subject to a permitted extension. Security, suppression, legal, and audit records may be retained where permitted or required.
